PHOTO & VIDEO CONSENT FORM (12-MONTH VALIDITY)
Oxford City Lacrosse Club is committed to ensuring the safety, privacy, and well-being of all our members,
players, and visitors. To promote the club, celebrate achievements, and share updates with our
community, we occasionally capture digital media during training sessions, matches, tournaments, and
official club events. Please indicate your consent to the Permissions we ask for below, or opt out, on your club registration form
Terms and Scope of Permission
Duration: This consent is strictly valid for a period of 12 months from the date of signature below. Upon
expiry, Oxford City Lacrosse Club will cease using your image in new promotional content. Archived
materials published during the active 12-month period (e.g., historical social media posts) may remain
online unless explicit removal is requested.
Usage Scope: Approved media may be used by club officials for marketing, informational, or promotional
purposes through the following official channels: the club’s verified website, private and public club social
media platforms, local news print, and digital club newsletters.
Rights & Withdrawal: You retain the right to withdraw this consent at any point during the 12-month
period by sending a written notice to the Club Secretary. Once received, the club will remove the relevant
digital images within 14 working days where reasonably practical.
